Individual Therapy
45 minute session / $200
Longer individual appointments available upon request and when clinically appropriate.
Couple Therapy
New couples wanting to establish themselves on my caseload must attend a 90 minute intake appointment ($375). This is an opportunity for me to get to know both partners and gather relationship history. Frequency and duration of ongoing sessions will be determined after this initial assessment.
Pre-Marital Counseling
Pre-marital counseling typically spans 3-6 sessions. Option of 45-, 60, and 75 minute sessions, depending on how quickly you would like to cover material. All pre-marital counseling couples are required to take the SYMBIS assessment prior to our scheduled intake appointment. Contact me to discuss the best plan of action for you and your loved one.
Insurance
I am not currently affiliated or “in-network” with any insurance panel. This is to ensure the greatest level of privacy, flexibility and control of your treatment and mental health records. All insurance claims require a diagnosis.
I am considered an “out-of-network” provider for all PPO insurance plans, though a portion or 100% of my fee may be covered after your deductible has been met. Clients are responsible for paying the full fee upfront for each session. I can provide an insurance receipt (“superbill”) at the end of each month for reimbursement purposes upon request. Please note that all clients are responsible for submitting their own claims directly to their health insurance provider as Moonwake Wellness does not offer insurance billing services. Insurance reimbursement is not guaranteed and is completely dependent on your coverage.

Do I have mental health benefits?
What is my deductible and has it been met?
How many mental health sessions per calendar year does my insurance plan cover?
How much does my plan cover for an out-of-network mental health provider?
How do I obtain reimbursement for therapy with an out-of-network provider?
What is the coverage amount per therapy session?
Cancellation Policy
If you need to cancel or make changes to a scheduled appointment please provide notification at least 24 hours prior to your appointment time via phone (text message/voice message) or e-mail. If the appointment is cancelled with less than 24 hours notice, you will be charged 50% of the session fee. All no shows will be charged the full session fee.
